Register to be a Coach/Team Parent/Dugout Parent

If I plan to be a head coach, assistant coach, dugout parent, team mom/parent or will be on the field in any way that I will fill out the google form link to register as either a head coach, assistant coach, dugout parent, team mom/parent. The inforamtion from this form will be used for a background check. This must be filled out every season. THIS FORM MUST BE COMPLETED BEFORE YOU STEP ONE FOOT ONTO THE FIELD.
